Position: Home Health Clinical RN- QA Specialist
Job Type: Full-time (40 hours/week)
Location: Remote, US-EST daytime hours
Salary: $8/hr - $9/hr

Overview

We are looking for a Home Health Clinical RN ? QA Specialist for an established home health organization that provides skilled nursing, physical therapy, aide-level services, and broader in-home care. The agency is strengthening its skilled home health operations and accreditation readiness while building a reliable clinical quality foundation to support continued growth and high standards of patient care.

This position will primarily focus on OASIS and clinical documentation quality review, ensuring patient records are accurate, complete, timely, and compliant with Medicare requirements. The RN will review the complete clinical record?including OASIS assessments, Plans of Care, physician orders, certifications, recertifications, and skilled documentation?to identify inconsistencies, compliance risks, and deficiencies before they result in survey or reimbursement issues.

The ideal candidate is an experienced Registered Nurse with extensive U.S. home health and OASIS QA experience who can serve as an independent clinical resource to leadership. This individual should possess strong clinical judgment, confidently identify and communicate documentation concerns, and help strengthen accreditation readiness, documentation standards, and quality improvement processes across the agency.

Key Responsibilities
Clinical Quality Review
- Review OASIS assessments for accuracy, completeness, and regulatory compliance.
- Perform comprehensive QA review of all clinical documentation prior to final approval.
- Verify physician orders, Plans of Care, certifications, and recertifications for compliance.
- Ensure documentation accurately supports skilled services and Medicare eligibility.
- Identify documentation deficiencies and recommend corrective action.
- Monitor chart completion and ensure documentation deadlines are met.

Accreditation & Regulatory Compliance
- Assist leadership with accreditation readiness and ongoing survey preparation.
- Review patient records to ensure compliance with Medicare Conditions of Participation.
- Help establish consistent documentation standards across the clinical team.
- Identify trends that may create compliance or survey risk.
- Maintain audit-ready patient records and supporting documentation.
- Assist with quality improvement initiatives and corrective action plans.
- Stay current on Medicare regulations, OASIS guidance, and home health best practices.

Clinical Documentation Support
- Review clinician documentation for accuracy, consistency, and completeness.
- Verify physician documentation supports the services being provided.
- Monitor unsigned orders, missing documentation, and expiring certifications.
- Assist with documentation corrections and follow-up with clinical staff.
- Support EMR documentation quality and workflow improvements.
- Ensure documentation is completed in accordance with agency policies and regulatory requirements.

Clinical Operations Support
- Serve as an additional clinical resource for leadership.
- Collaborate with intake, scheduling, and field clinicians to resolve documentation issues.
- Help improve clinical workflows and operational efficiency.
- Provide guidance regarding documentation best practices.
- Maintain strict HIPAA compliance and patient confidentiality.
- Perform additional clinical QA responsibilities as assigned.

Preferred Skills & Qualifications
- Registered Nurse (RN) required.
- Minimum 5 years of U.S. home health experience.
- Extensive OASIS QA experience.
- Strong understanding of Medicare Conditions of Participation.
- Previous accreditation, CHAP, ACHC, or Joint Commission survey experience preferred.
- Experience reviewing physician orders, Plans of Care, and home health documentation.
- Excellent critical thinking and clinical judgment.
- Strong attention to detail.
- Excellent written and verbal communication skills.
- Ability to work independently with minimal supervision.

Bonus / Nice-to-Have Skills
- Experience helping agencies prepare for Medicare certification or accreditation.
- Previous Clinical Manager, QA Manager, or Director of Clinical Services experience.
- Bilingual English/Spanish.
- Experience with WellSky, Axxess, Homecare Homebase, MatrixCare, or other major home health EMRs.
- Experience mentoring clinicians on documentation quality.
- Familiarity with quality improvement and performance improvement initiatives.
